Secure Member Access
Access secure benefit information for plan members and covered dependents. View your benefits summary, pending and paid claims, pretreatment estimates, and status of remaining plan benefits.
• Log in with your User ID and password.
• Register Now to create a User ID and password.
• Your secure account contains Protected Health Information. To complete your account registration and access the secure site, you’ll need the member’s Social Security Number/Member ID or the private source code that can only be found on a benefit statement (claim explanation) that you have received in the last 15 months.

Register for secure access to eligibility/benefits and in-depth claim information for all of your patients and covered dependents insured with us.
• Verify patient benefits.
• View your insured patients' dental and/or orthodontic claims for the past 36 months.
• Confirm payment amounts and check dates for processed claims.
• Access your "Resource Center" with claim forms, information on submitting claims, electronic claims, finding a dental network provider for referrals, joining our network and interpreter services for patients.
New Users will need:
• A recent Explanation of Payment (EOP) statement.
• The Tax Identification Number (TIN) or Social Security Number (SSN) used when submitting claims.
• The phone number you provide on claims forms.
